Little Princess Spa focuses on having small intimate groups with one-on-one attention for all our girls to make their experience that much more enjoyable!
RELEASE OF MEDICAL INFORMATION AND PERMISSION FOR EMERGENCY MEDICAL TREATMENT
I authorize Little Princess Spa to obtain from my physician information concerning my child. I authorize the following individuals to have access to my child’s care and records, in case of emergency only. I also authorize Little Princess Spa to obtain all necessary care for my child in case of an emergency. In the event of an emergency or illness, I will arrange for my child to be picked up from Little Princess Spa within one hour after I am notified. I furthermore agree to notify Little Princess Spa if my child has been exposed to a communicable (contagious) disease.
HEALTH CERTIFICATION
This is to certify that my child(ren) is in good physical condition and has no recent exposure that would be contagious for others and has my permission to participate in the total program. In case of emergency, I understand every effort will be made to contact the parents and guardian of the camper. In the event I cannot be reached, I hereby give permission to Little Princess Spa to provide whatever emergency measure as judged necessary for the care and protection of my child while under their supervision on premises. In case of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment, if local emergency resources deem it necessary. It is understood that in some medical situations, the staff will need to contact local emergency resources before the parent, child’s physician, and/or adult acting on the parent’s behalf. In addition, I certify that my child has had no operations or serious illness between her last health examination and the opening of the camp session.
PARENT AGREEMENT
This letter outlines important camp information, and your signature below means you will comply with the statements included.
I understand:
That I give my child permission to participate in all approved, scheduled activities in camp.
All activities are governed by sound health and safety standards.
I will make full payment and complete all required paperwork by 6pm on Monday, one week prior to the start of each week. If payment is not received by this deadline, registration will automatically be canceled and a child from the waiting list will be called.
No refunds will be made after the due date without a written statement from a licensed physician.
I understand that supplies, etc. have been purchased.
In the event of an emergency, every effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to the adult in charge to secure emergency treatment for my child.
My family health insurance will provide primary coverage for such medical treatment.
My child may participate in any media related photographs or taping for airing or printing.
I hereby give Little Princess Spa the right and permission without compensation to use photographs of my child and their name for public relations purposes.
Little Princess Spa will not be responsible for loss of valuables. I will label my child’s personal items.
Little Princess Spa, Inc will not be responsible for any slip or fall accidents and/or any allergic reaction to food or products.
My child will abide by all camp rules and regulations.
Proper behavior is required and there will be no refund if my child should be sent home due to misbehavior, or behavior which represents a danger to themselves or to others.
I confirm that my child is fully potty trained and is no longer in diapers.
Camp starts and ends on time. A $5.00 fee per minute will be charged if child(ren) is not picked up on time.
Parent/Guardian please make sure you sign your child in and out daily!
PHOTO RELEASE ACKNOWLEDGEMENT
I, being Parent/Guardian of the child listed here hereby consent that my Child’s name, image, and likeness, as shown in video-tapes, photographs, film and/or electronic images for which they are posed, and/or audio recordings made of their voice may be used by Little Princess Spa, Inc. its assigns or successors, in whatever way they desire, including television; furthermore, I hereby consent that such photographs, films, recordings, electronic images, and the plates, tapes and/or software from which they are made shall be their sole property, and they shall have the right to sell, duplicate, reproduce and make other uses of such photographs, films, recordings, electronic images, plates, tapes and software as they may desire free and clear of any claim whatsoever on my part.

Pack Extra Clothes for Emergencies. Place it all in a Ziplock Bag with Camper’s Name.

**We recommend leaving all cellphone and other electronic devices at home as this will help your child to participate and interact during their stay.
